Daily Form keeps your health data on your iPhone. There is no account, no server, no analytics, no ads and no tracking. Your data is worked out on your iPhone and never leaves it unless you choose to share an export. The developer never receives any of your data.
With your permission, Daily Form reads (and never writes) data from Apple Health: sleep, heart rate, heart-rate variability, resting heart rate, breathing rate, blood oxygen, wrist temperature, workouts and their effort ratings, activity (such as steps, energy, exercise and daylight), other readings shown in “Your body”, State of Mind, and your age and sex. If you turn on cycle-aware insights, it also reads your period logs. You can change these permissions at any time in the Health app.
Daily summaries (your scores and the readings behind them, for the last 8 weeks) so the app opens instantly: encrypted by iOS, readable only while your iPhone is unlocked, and excluded from iCloud and computer backups. Raw Apple Health samples and period data are never saved. A small summary for the widgets is kept in your iPhone’s keychain, on this device only. Your check-ins, notes and settings are stored in the app’s preferences. You can delete the summaries, check-ins and notes in You › Your data › Delete saved data; deleting the app removes everything it stored.
It never sends your data to the developer or anyone else, never stores health data in iCloud, never uses your data for advertising and never sells it.
On iPhones with Apple Intelligence you can ask questions about your data. Answers are worked out by Apple’s on-device model on your iPhone from a short summary of your numbers. The conversation isn’t saved and nothing is sent to the developer.
Data leaves your iPhone only when you share it yourself: “Export my data” creates a spreadsheet for the share sheet, and “Send feedback” creates a note with app versions, settings and how many days of data exist (never health readings). Export files are deleted when you leave that page. Reminders are scheduled on your iPhone and can show your verdict and scores on the Lock Screen; you can turn numbers off in You › Reminders. Daily Form isn’t directed at children under 13.
